How to treat Eczema? – Dr. Jagadish Puttarangappa

The treatment options for eczema depends on causes of eczema. Suppose the eczema is related to some metals we need to avoid that metals. If eczema is related to some food stuff or coming in contact with soaps and detergents we have to totally avoid. Simple water can leads to actually constant contact with water or soaps definitely the skin is prone to develop these kind of eczemas. Those conditions we advised them stay away from these stimulants. Other things in are eczema suppose if the eczema is in atopic dermatitis. So in atopic dermatitis we have to avoid a lot of things means it depends on each individuals we cannot say the same options or the same kind of treatment or modalities to each and every one because each eczema is an individualized thing. So accordingly we need to the treat the patient. Supposed eczema is an atopic dermatitis we have to avoid them playing with mainly domestic animal like pet dogs or cats because those pet dogs and cats will have the fur which will actually have lot of microorganisms which enters the skin and it stimulates the immunological reactions and lead to eczematous changes, so that is one thing. Suppose eczema is secondary to cold even in atopic dermatitis some patients get exposure to extremes of cold definitely they start getting lot of inflamed itchy lesions. So in those conditions we should advise them to use lot of moisturizers, advise them to keep their body warm and not to expose to this extreme colds. So these are some of the treatment modalities in eczema. Suppose if patient many time we are seen patient having eczematous changes to sometimes even bike when they ride, in the hands we do find lot of eczematous changes because of either because of the rubber what they constantly come in contact or because of the metal next to the handle. So in those conditions we should advise them to use some pure cotton gloves and then ask them to ride a vehicle. So that will actually relieve them lot along with that tell the existing problem need to be treated with moisturisers, with steroids. Steroid of course we cannot just take it and start using it, because it need to be prescribed by the physician because steroids are like double wedge weapon if it used properly it 100% works and there is no side effect. If it is adversely I mean without prescription if you start using this definitely it can cause lot of adverse events. So you cannot blame just on the steroid. Most of the time it is the patient themselves or without proper instructions if they have used it, then it can lead to side effects, so that is one thing. The eczema involves entire body sometimes. Suppose the flexors and sometimes extensors, sometimes the scalp, sometimes the intertrigenous areas. Depending on these conditions we need to define what kind of medications the patient need to use. We cannot just ask them to use some creams, no. So if patient got thick plaque, scaly lesions or eczematous changes on the limb or arms or back then you have to use ointment based moisturizers. If they have got mild lesions then we need to use creams. If they have got, for example in intertriginous area or in the scalp, in those conditions we may have to use moisturisers with lotions. On the same basis we use steroids. Steroids come in different formulations. So depending on the area involved, we advise and of course the potency, there are different potency of steroids. So there are mild, mid, supra and super potent steroids. So these steroids are used on the conditions and the site involved. So you need to consult a dermatologist for this because the dermatologist is the best people to analyse and give you a proper treatment for these conditions.
