Scalp psoriasis can be difficult to treat, but consistent topical care, even at home, can make a significant difference. Here’s what I recommend:
1. Use a 3% salicylic acid shampoo (like Neutrogena T/Sal). Salicylic acid is a keratolytic, so it is going to help breakdown plaques and scalp scaling. Leave it on for 5 minutes before rinsing off.
2. Alternate between salicylic acid shampoo and Nizoral shampoo on wash days. Nizoral shampoo contains ketoconazole 1%, which is an anti-fungal agent that can help reduce scalp itch and helps control yeast overgrowth that is responsible for some scalp psoriasis flares. Washing frequency is based on hair texture, but washing your hair more frequently generally improves scalp psoriasis.
3. Use mineral oil on your scalp as a mask at least weekly. Mineral oil can help soften and loosen plaques so that they come off more easily and can also help medicated shampoos work better. Apply it directly to your scalp in sections, and leave it on for one hour to overnight before washing off.
For moderate to severe scalp psoriasis, seek treatment from a board-certified dermatologist.
