One of the most common diseases that a skin specialist encounters is psoriasis. What is psoriasis? Psoriasis is an inflammatory condition which is caused by body’s immune system . So we call it as an autoinflammatory condition and it’s also called a disorder of keratinization that is disorder of formation of the skin. So there is a defect in the formation of skin cells skin cells are produced at a very rapid rate and they are not matured properly. So therefore they may have cracks cuts and because of rapid production of skin. There is flakes are produced or scales are. So the hallmark of a psoriatic lesion is flakes or scales. So whenever if you notice on your head or on your hands or on your legs or back or anywhere in the body you have patches which have scales. Scales are nothing but a dead skin kind of powder white colour powder coming out of the scales. The patch may be black or red so in very dark skinned people the patch may look black in more fairer skin people the patch may look great. So this is because of the inflammation. Psoriasis is a long-term problem and it’s a chronic disorder. So we need to have regular maintenance with a dermatologist. There are various medicines available to treat psoriasis for very simple single patch or few patches of psoriasis usually which is less than 10 percent of the body’s surface area. Use topical treatment topical means creams or lotions or ointments and what is applied on the skin. For little more multiple patches of psoriasis we use internal medicines. So there are group of medicines called as immunomodulators or immunosuppressants. So these are used these have to be monitored very carefully. So qualified dermatologist has to prescribe this by monitoring your liver,kidney etc. A new group of medicines called as biologics are available to treat these kind of autoinflammatory disorders. So biologics can treat conditions like rheumatoid arthritis psoriasis and other autoinflammatory diseases which produce certain chemicals causing the inflammation. Advantage of biologics is that they specifically target these chemicals. They are usually made up of antibodies. So it doesn’t affect liver kidney etc and many of them are similar to human antibodies. So the side effects are less however these can also affect our immune system and there can be other effects of biologics. So therefore infections etc have to be monitored more carefully when using biologics. So if you have red or dark colored patches with lot of flakes or scales it may be psoriasis. Commonest area of involvement is a scalp other areas can be hands legs back etc and it can involve any part of the body. Some people have nail psoriasis as well and a small percentage of psoriasis patients have arthritis. So psoriatic arthritis where there will be swelling of the joints. Usually the small joints in the fingers will be affected. Fingers and toes will be affected. Those who have arthritis definitely require internal medication either immunomodulators or biologics.
